RN/LPN - Per Diem Overnights

Per Diem RN or LPN sought for Overnight coverage at RiverWoods in Hampton, NH. Provides direct resident care in a team-focused environment, collaborating with interdisciplinary team members to formulate and update individualized care plans. Implements care protocols (falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain management). Assesses and documents changes in residents’ status, reports findings to the care team, and supports leadership by coordinating daily assignments and supervising direct caregivers. Administers medications and treatments per professional guidelines and nursing department policies, maintains narcotics security, and upholds standards under the NH Nurse Practice Act, OBRA, and HIPAA.
