Job Summary
Conduct direct resident care in a team-focused environment, including medication administration, falls prevention, and infection control. Collaborate with interdisciplinary members to formulate and update individualized care plans, while assessing and documenting changes in residents' physical or mental status. Provide leadership to direct caregivers by preparing daily assignments and delegating workload through observation and evaluation. Maintain positive relationships with team members, residents, and families to facilitate open communication. Obtain, administer, and document treatments following professional guidelines and department policies.
Required Qualifications
- Nursing Degree or Diploma from an accredited college, University or nursing school
- Registered nursing (RN, LPN) license within the State of New Hampshire
- CPR certification
- Maintain CEU's annually
Desired Qualifications
- Long term care facility experience
- Specialized knowledge of nursing care of the elderly
- Rehabilitative nursing knowledge
- Certified in intravenous administration
