Job Summary
RN/LPN Home Care Coordinator role providing care coordination within the PACE interdisciplinary team, planning and evaluating home care services, conducting home visits, authorizing services, and maintaining timely documentation. Responsibilities include reviewing assessments, scheduling rounds, collaborating with OT, working with contracted vendors, updating care plans, performing nursing assessments, ensuring quality of care, and participating in family meetings. Requires MA RN licensure, CPR, 2+ years in geriatric and community health settings, and ability to travel locally. Full-time, Monday–Friday 8–4, with rotating after-hours on-call; no nights/weekends/holidays. 80k-104k for RN and 74k-88k for LPN depending on role, with benefits. Must be able to pass fit test (facial hair restrictions for respirator). EEO employer.
Required Qualifications
- Successful Completion of an Accredited RN Program
- Current Licensure as a Registered Nurse in the Commonwealth of Massachusetts
- Current CPR certification
- A minimum of 2 years of experience working with a geriatric population
- A minimum of 2 years of working in a community health setting
- Flu shot required
- Covid vaccine preferred
- Bachelor of Science Degree in Nursing, preferred
