Job Summary

Provide professional nursing care for residents of all ages in assisted living under the supervision of a registered nurse or physician. Demonstrate competency in age and population-specific standards for physical and psychosocial needs, perform testing to evaluate resident status, and administer medications or treatments as ordered. Ensure resident health, comfort, and safety while documenting interactions, outcomes, and care plans. Counsel residents, family members, and advocates on prevention and treatment plans, and establish effective relationships with interdisciplinary care teams. Requires current licensure as a practical nurse in the state of practice.

Required Qualifications

  • Must have graduated from an accredited school or college of nursing program leading to licensure as a Licensed Practical Nurse
  • Requires current licensure as a practical nurse in the state/states of practice
  • Demonstrate knowledge of nursing principles for the resident in the relevant clinical setting
  • The LPN demonstrates competency and practices within the full scope of nursing expertise/knowledge
  • utilizes appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan
  • Must possess the ability to perform testing to evaluate data reflective of the resident's status
  • Ensures the health, comfort and safety of residents
  • Contributes to the assessment of residents
  • administers medications or treatments as ordered
  • Provides technical support to healthcare professionals as needed
  • Documents resident interactions as needed and the outcomes related to resident care, medication, behavior, activity, and dietary
  • Skilled in establishing and maintaining effective working relationships with resident, health care providers, and the public
  • Ability to work effectively with professional and supportive personnel as a member of an interdisciplinary care team
  • Counsels residents, family members and/or resident advocate, under the direction of a registered nurse, advanced practice provider or physician on prevention and treatment plans
  • Knowledge of resident education principles to develop a self-care program
  • Must be able to lift 50 lbs
