Job Summary

Assess physical, psychosocial, and spiritual needs of patients and families, gather related data, and provide intermittent skilled nursing services under physician direction. Monitor and evaluate pain and symptoms, offer emotional support, and relay pertinent information to the physician and team members. Serve as a liaison to coordinate care, provide health care teaching for residence-based management, and make visits to patient residences, including at the time of death. Maintain all required documentation promptly and participate in mandatory or elective team meetings and trainings. Work the 8:00p-8:30a, 36-hour every other weekend schedule with a $3,000 sign-on bonus.

Required Qualifications

  • High School Diploma or equivalent
  • Licensed RN/LPN in state of Ohio
  • Valid driver's license
  • Good driving record
  • Auto accident insurance at a level acceptable to HWR and/or HWR's insurer
  • Active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Active driver's license
  • Ability to provide proof of eligibility to work in the United States
  • High level of integrity, ethics and professionalism
  • Ability to stand and walk for extensive periods of time
  • Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment
  • Ability to stoop, squat, or bend frequently
  • Corrected vision and hearing within normal range to observe and communicate with patients and professional staff
  • Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow filled walkways, etc.)
  • Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere
  • Must have demonstrated knowledge and skills to provide appropriate care to all age groups
  • Ability to communicate effectively, orally and in writing
  • Ability to work independently and collaboratively with a team
  • Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology
  • Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer

Desired Qualifications

  • Graduate of an accredited school of nursing, with a Bachelors degree preferred
  • Experience preferred in pain and symptom management, oncology, or mental health
