Discuss resource utilization, treatment plans, and discharge strategies with physicians, focusing on complex patients, frequent ED utilizers, and chronic conditions. Ensure the interdisciplinary care plan remains consistent with covered services by overseeing data collection for regulatory and accreditation compliance. Manage frequent ED visitors through focus study reviews of admissions and coordinate patient transfers to tertiary centers. Requires RN licensure and BSN; full-time day shift in Montebello.
