RN - Lead Care Manager (Full-Time, Day Shift)

Lead the coordination of patient care with interdisciplinary teams, monitoring diagnostic testing, treatments, and procedures to ensure clinical appropriateness. Discuss resource utilization, consultations, treatment plans, and discharge plans with physicians, focusing on complex patients, frequent ED utilizers, and chronic conditions. Collaborate on ongoing care, coordinate transfers to tertiary centers, and provide oversight for regulatory and accreditation compliance. Manage frequent ED visitors through focus study reviews of admissions. Requires RN licensure and BSN preferred. Full-time day shift position at Adventist Health White Memorial Montebello.
