RN - Lead Care Manager (Full-Time, Day Shift)

Lead the coordination of patient care with interdisciplinary teams, monitoring diagnostic testing, treatments, and procedures to ensure appropriateness and timeliness. Discuss resource utilization, consultations, treatment plans, and discharge strategies with physicians, focusing on complex patients, frequent ED utilizers, and chronic pain cases. Ensure the interdisciplinary care plan aligns with clinical courses and covered services while overseeing regulatory and accreditation compliance data collection. Manage frequent ED visitors through focus study reviews of admissions and coordinate patient transfers to tertiary centers. Requires RN licensure, BSN preferred, and experience in care management roles.
