RN - Lead Care Manager (Full-Time, Day Shift)

Lead the coordination of patient care with multidisciplinary teams, monitoring diagnostic testing, treatments, and procedures to ensure clinical appropriateness and timeliness. Discuss resource utilization, consultations, treatment plans, and discharge strategies with physicians, focusing on complex, frequent ED utilizer, and chronic pain patients. Collaborate with acute care case managers, oversee patient transfers to tertiary centers, and collect data for regulatory and accreditation compliance. Manage frequent ED visitors through focus study reviews of admissions. Requires RN licensure, BSN preferred, and experience in care management. Full-time day shift at Adventist Health White Memorial Montebello.
