RN Intake Coordinator

RN Intake Coordinator role coordinating with clinical management and physicians' offices to ensure proper clinical care, communicating lab results and patient status with MD offices, answering incoming calls to gather and process referral information for initial assessments, obtaining referral details to transition to homecare (demographics, diagnoses, insurance, orders, caregivers, F2F documentation), and assisting in scheduling patient services. Strong communication, attention to detail, and customer service are emphasized as part of joining the Visiting Nurse Association of Florida team.
