RN - Inpatient Heart Failure, FT WEO

Assess, plan, implement, and evaluate patient-centered care for heart failure patients within the Integrated Practice Unit. Coordinate patient care activities and manage individualized transition planning to reduce readmissions and improve symptom control. Collaborate with the interdisciplinary team to uphold standards of nursing practice and ensure safe, effective, and high-quality care across the continuum. Requires current Georgia RN license, BLS certification, and eligibility as a new graduate. Full-time, evening shift position at Piedmont Athens Regional Hospital.
