Job Summary
RN Assessor conducts in-home assessments for PPP, medical daycare and personal assistance programs in Camden County with ~60% field-based and ~40% remote work. Responsible for evaluating ADLs/IADLs, observing and interviewing to determine functional needs, maintaining documentation, coordinating with physicians, hospitals, and providers, and connecting members to case management or long-term support services as appropriate. Manages own schedule to meet a 7-day turnaround for evaluations, completing ~14–16 assessments per week. Requires nursing licensure in New Jersey and strong clinical judgment, documentation, and communication skills; travel is local daily. Role includes administrative duties such as authorization completion, scheduling follow-ups, and ensuring care is medically appropriate per guidelines. Required Qualifications
- Active Unrestricted NJ RN License Required
- Requires an associates or bachelors degree (or higher) in nursing
- Minimum of two years clinical experience preferred
