Job Summary
Accept Hospice, Home Health, and Home Infusion referrals in acute care, physician clinics, nursing homes, and rehabilitation centers. Maintain an active presence in these settings to deliver customer service to discharge planners, medical social workers, physicians, and future patients. Assist in the facilitation of timely discharges and support system initiatives for continuity of care within the Mercy Health System. Schedule is 8am-4:30pm PRN. Requires current RN license, bachelor's degree preferred, and minimum three years of nursing experience.
Required Qualifications
- Registered Nurse
- Current RN license in applicable state of practice
- Minimum three years of nursing experience

- Experience Hospice and Home Health
- Experience in the in-patient acute setting
- Experience in the community setting
- Experience in providing Hospice/Home nursing care
- Certification in Hospice and Palliative Nursing (CHPN)
