Job Summary
Accept hospice, home health, and home infusion referrals across acute care, physician clinics, nursing homes, and rehabilitation centers. Maintain an active presence in these settings to deliver customer service to discharge planners, medical social workers, physicians, and future patients. Assist in facilitating timely discharges and support system initiatives for continuity of care within the Mercy Health System. Work 8am-4:30pm PRN hours with a focus on hospice and palliative nursing care.
Required Qualifications
- Registered Nurse
- Current RN license in applicable state of practice
- Minimum three years of nursing experience
Desired Qualifications
- Bachelor's degree
- Experience Hospice and Home Health
- Experience in the in-patient acute setting
- Experience in the community setting
- Experience in providing Hospice/Home nursing care
- Certification in Hospice and Palliative Nursing (CHPN)
