RN Hospital Community RN Liaison - Hospice - PRN

Job Summary
Accept Hospice, Home Health, and Home Infusion referrals from acute care, physician clinics, nursing homes, and rehabilitation centers. Maintain an active presence in these settings to deliver customer service to discharge planners, medical social workers, physicians, and future patients. Assist in the facilitation of timely discharges and support system initiatives for continuity of care within the Mercy Health System. Perform nursing assessments, communicate services to post-acute communities, and provide timely written documentation. Shifts are 8am-4:30pm PRN. Current RN license and three years of nursing experience are required, with hospice and acute setting experience preferred.
Required Qualifications
- Registered Nurse
- Bachelor's degree
- Current RN license in applicable state of practice
- Minimum three years of nursing experience
Desired Qualifications
- Experience Hospice and Home Health
- Experience in the in-patient acute setting
- Experience in the community setting
- Experience in providing Hospice/Home nursing care
- Certification in Hospice and Palliative Nursing (CHPN)
- Complies with accepted professional service standards and practice
- Understands and is supportive of the philosophy of Hospice and Home Services
- Demonstrates understanding of current guidelines, regulatory and clinical in nature
- Assists in determining client appropriateness for Hospice and Home Services
- Ability to communicate services to physicians and post-acute communities
- Excellent observation, nursing assessment and communication skills
- Ability to provide written documentation in a timely manner
- Self-directed with the ability to work with little supervision
- Flexible and cooperative in fulfilling role obligation
- Ability to work within a team
