Job Summary
RN Hospital Community Liaison for Hospice - intake coordination across Hospice, Home Health and Home Infusion referrals in acute care, physician clinics, and select nursing homes; maintains active presence in these settings; delivers customer service to discharge planners, medical social workers, physicians, and patients; facilitates timely discharges and supports continuity of care within Mercy Health System; hours 8am-4:30pm, 40 hours/week with weekend rotation; benefits include tuition reimbursement, health insurance, PTO, retirement plan, and more; required RN license, 3+ years nursing experience with hospice/home health preferred; CHPN certification a plus; strong communication, documentation, and teamwork skills.
Required Qualifications
- Registered Nurse license in applicable state
- Bachelor's degree preferred
