Job Summary
Perform comprehensive in-home patient assessments, monitor vital signs, and administer medications including IV therapy and wound care. Collaborate with physicians and interdisciplinary teams to coordinate labs and diagnostics, while educating patients and caregivers on disease management and warning signs. Maintain accurate EMR documentation in real time, follow infection prevention protocols, and act quickly to initiate interventions or coordinate hospital transfers during emergencies. Deliver hospital-level care in a 1:1 setting with flexible morning, night, or all-shift options.
Required Qualifications
- Active Registered Nurse (RN) license (Ohio)
- Minimum 1-2 years of RN experience
- Local candidate within 30 miles of Cleveland area
- Valid driver's license
- Reliable transportation
- Auto insurance
- Willingness to travel between patient homes
- Current CPR certification
- Negative TB test (within 12 months)
- Up-to-date immunizations or exemptions (COVID, Flu, MMR, Hep B)
- Ability to pass background check and drug screening
- Strong clinical judgment and critical thinking
- Ability to work independently in a home setting
- Excellent communication and patient education skills
- Tech-savvy (EMR, mobile charting, telehealth tools)
- Compassionate, adaptable, and patient-focused
Desired Qualifications
- Experience with IV therapy, wound care, or acute care
- Experience in home health, ER, ICU, or med-surg
