Job Summary
Oversee weekend hospice clinical operations remotely, providing weekend leadership and coordination of hospice care. Lead and support an interdisciplinary team, manage referrals, admissions, recertifications, discharges, and crucial patient care interventions, and ensure regulatory compliance and high-quality documentation. Drive workflow optimization, triage and clinical decision support, and effective handoffs to weekday leadership, while maintaining strong communication with patients, families, and referral sources in a remote setting.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license
- Minimum of 3 years of hospice nursing experience
- Minimum of 1 year of leadership, supervisory, or care coordination experience
- Strong knowledge of hospice regulations, Medicare guidelines, and end-of-life care practices
- Experience using electronic medical record (EMR) systems
- Excellent communication, critical thinking, and organizational skills
- Ability to work independently in a remote environment
- Reliable internet connection and dedicated home workspace
