Job Summary
Hospice Case Manager RN coordinates comprehensive home-based hospice care, evaluating patient needs, developing and implementing individualized care plans, and coordinating care across an interprofessional team with patient, family or caregiver involvement. Requires strong assessment and IV skills, ability to work within the BJC Home Care program, and commitment to advancing through the BJC RN Career Ladder. Responsibilities include assessing patient preferences, documenting care plans, communicating across the continuum, promoting patient- and family-centered care, ensuring evidence-based nursing care, and maintaining EMR updates. The role involves a 40-hour work week with weekend rotation, on-call nights, and potential sign-on bonus and career progression. Safety-sensitive position with licensure requirements and willingness to provide coverage across St. Louis regions.
