Job Summary
Respond to clinical referral information with care, assessing client eligibility for specialized hospice services and guiding patients and families through their end-of-life journey. Deliver hands-on nursing care rooted in clinical principles while coordinating services and educating families to create personalized care plans. Share call duties as part of the interdisciplinary team to ensure continuous support. Requires current unencumbered RN license, one year of acute care experience, and EMR proficiency. Per Diem schedule available in Casa Grande.
Required Qualifications
- Hold current unencumbered license as a Registered Nurse
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Must be computer proficient in typing and various programs, including background in EMR
- Possess and maintains current CPR certification if required by state
- Current driver's license
- Vehicle insurance
- Access to dependable transportation
