RN, Hospice- PRN

Administer prescribed medications, perform nursing care, and initiate treatment plans for hospice patients upon admission and during re-evaluations. Prepare clinical progress notes, teach families regarding care needs, and supervise licensed practical nurses and home health aides on-site. Serve as primary case manager to coordinate services, conduct supervisory evaluations every 30 days, and certify patients for hospice criteria. Take clinical calls as needed, communicate changes to physicians, and function as patient advocates.
