RN, Hospice- PRN

Administer prescribed medications and treatments, perform nursing care according to patient needs, and initiate clinical evaluations upon admission to assess physical, functional, and psychosocial requirements. Develop and update treatment plans, supervise licensed practical nurses and home health aides, and conduct on-site supervision every 14 days to assess care goals. Serve as primary case manager to coordinate services, provide written instructions for personal care, and certify patient eligibility for hospice services. Take clinical call as needed and function as patient advocate in all decisions affecting care. Requires Texas RN license with 2 years clinical experience; PRN shift in La Grange, Texas with 403b retirement matching.
