Job Summary
Respond to clinical referral information with care, assessing client eligibility for specialized hospice services. Partner with the Clinical Director to guide care planning, ensuring patients and families receive comfort, dignity, and personalized support aligned with their goals. Deliver hands-on nursing care rooted in clinical excellence while sharing call duties to provide continuous presence during critical moments. Empower families through education, encouraging active participation in creating care plans that reflect their values.
Required Qualifications
- Hold current unencumbered license as a Registered Nurse
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Must be computer proficient in typing and various programs, including background in EMR
- Possess and maintains current CPR certification if required by state
- Current driver's license, vehicle insurance, and access to dependable transportation
- Schedule: PRN - Weekdays and Weekends 8 am - 4:30 pm
- Territory: Tri County - Dorchester, Charleston and Berkeley
