Job Summary
Responsible for direct patient care and case management of Hospice patients in outpatient settings. Ensures that end-of-life care is delivered within quality and utilization guidelines and directed toward achieving desired clinical outcomes for patients and families. Develops, coordinates, and schedules delivery of care through a multidisciplinary team to patients in the home or community; assures completeness and timeliness of the patient care plan and its implementation with ongoing evaluation. Maintains documentation for regulatory requirements and reimbursement, while serving as a patient advocate and precepting newly hired nurses. The role includes coordination with the multidisciplinary team, support services, and community resources to meet patient needs, and may involve holiday/weekend coverage as needed. Requires an Associate Degree in Nursing and active RN license in NM, with BLS at minimum, and one year of direct patient care experience. Compensation range: $37.80-$51.00 per hour; full-time schedule in Days shift.
Required Qualifications
- Associate Degree in Nursing
- Licensed Registered Nurse in NM (RN-NM)
- Minimum one year in direct patient care setting or home health care
- Current driver's license and auto insurance
- BLS required
- Education: Associate Degree in Nursing
