Job Summary
Direct patient care and case management for hospice patients in outpatient/home settings. Coordinates and delivers care through a multidisciplinary team, ensuring timely and complete care plans, regulatory-compliant documentation, and communication with the team and community resources. Precepts newly hired nurses and promotes professional development, QAPI/PIP participation, and continuity of care. Requires RN licensure in NM, at least one year of direct patient care/home health experience, BLS, and driving privileges. The role emphasizes development of care plans, coordination with physicians and care givers, and ensuring cost-effective delivery of hospice services.
Required Qualifications
- Associate Degree in Nursing
- Registered Nurse-NM
- Minimum one year in direct patient care setting or home health care.
- License to practice as Registered Nurse in the state of New Mexico
- Current drivers license and auto insurance.
- BLS required
