Job Summary
Hospice Nurse responsible for overseeing the quality, expert care of assigned hospice patients; perform patient assessments, establish and implement individualized care plans, provide education to patients and families, obtain physician orders for symptom management, triage and arrange home visit schedules, and collaborate with IDT members including social workers.
Required Qualifications
- Minimum of two years of nursing experience
- NJ RN license required; BSN preferred
