Job Summary
Conduct comprehensive patient assessments to determine physical, emotional, and psychosocial needs for individuals facing life-limiting illnesses. Develop, implement, and regularly update individualized care plans tailored to patient goals and comfort. Administer medications and treatments as prescribed while monitoring responses and adjusting care accordingly. Provide education and emotional support to patients and families regarding disease progression, symptom management, and end-of-life planning. Collaborate closely with interdisciplinary teams including physicians, social workers, chaplains, and volunteers to coordinate holistic care. Document all patient interactions, care plans, and clinical observations accurately and timely in compliance with regulatory standards. Participate in quality improvement initiatives and maintain up-to-date knowledge of hospice care best practices and regulations.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the United States
- Minimum of one year of nursing experience
- Basic Life Support (BLS) certification
- Strong clinical assessment and critical thinking skills
- Ability to work independently and manage a caseload effectively
Desired Qualifications
- Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification
- Experience with electronic medical records (EMR) systems
- Advanced certifications such as Advanced Cardiac Life Support (ACLS)
- Experience in patient and family education and counseling
- Familiarity with Medicare and Medicaid hospice regulations and documentation requirements
