Job Summary
Assess, plan, implement, coordinate, monitor, and evaluate strategies to meet end-of-life care needs for patients and families with a life expectancy of six months or less. Develop comprehensive plans addressing physical, emotional, cultural, and spiritual requirements while advocating for pain and symptom management. Direct and supervise LPNs and CNAs, administer medications as ordered, and respond to emergency situations. Attend shift reporting and interdisciplinary team meetings to facilitate communication regarding patient care decisions. Complete all position and unit safety competencies on a timely basis. Full-time day shift (10:30 a.m. to 11 p.m.) with variable days and every third weekend off.
Required Qualifications
- Associate's degree in nursing
- 1 year of relevant experience preferably in Hospice
- Michigan Registered Nurse (RN) license upon hire
- CPR, (AHA) American Heart Association within 90 days of hire
- Exhibit 'Patients and Families First' attitude
- Responds with compassion, flexibility and urgency to each patient's needs
- Develop a comprehensive plan of care that addresses physical, emotional, cultural and spiritual needs of the patient/family
- Advocate for pain/symptom management
- Direct and supervise LPN and CENA
- Administer and document medications/treatments as ordered by the physician according to policies in an accurate and timely manner
- Identify and respond appropriately to emergency situations
- Attend shift reporting and IDT meetings as necessary to facilitate communication of decisions related to the patients care
- Actively participates in safety initiatives and risk mitigating measures where appropriate
- Complete all position and unit safety related competencies and requirements on a timely basis
- Comply with the MMR, Varicella, Tdap, and Influenza vaccine requirement
