Job Summary
Coordinate patient and family care by conducting continuous assessments and implementing interdisciplinary care plans per physician orders. Ensure all services comply with federal and state regulations while maintaining basic technology skills for mobile device operation. Maintain a valid driver's license, automobile liability insurance, and dependable transportation to safely drive in all weather conditions. Hold current CPR certification for healthcare providers. Medicare home health or hospice experience is preferred, and education requirements may be deferred with leadership approval.
Required Qualifications
- Must be a graduate of an approved school of clinical education
- Must be licensed in the state where they currently practice
- Must have at least one year experience as a licensed professional
- Must have basic demonstrated technology skills, including operation of a mobile device
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- Must possess CPR certification for the healthcare provider
Desired Qualifications
- Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership
- Medicare home health or hospice experience is preferred
