Job Summary
Coordinate patient and family care by conducting continuous assessments and implementing interdisciplinary care plans in accordance with physician orders. Deliver services compliant with federal and state regulations while utilizing approved mobile devices for technology operations. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation to safely drive in all weather conditions. Hold current CPR certification for healthcare providers and possess at least one year of licensed professional experience. Medicare home health or hospice experience is preferred, with education and experience subject to review by clinical leadership. Required Qualifications
- Must be a graduate of an approved school of clinical education
- Must be licensed in the state where they currently practice
- Must have at least one year experience as a licensed professional
- Must have basic demonstrated technology skills, including operation of a mobile device
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- Must possess CPR certification for the healthcare provider
Desired Qualifications
- Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership
- Medicare home health or hospice experience is preferred
