Job Summary
Coordinate patient and family care needs by implementing interdisciplinary care plans in accordance with physician orders and the established plan of care. Ensure continuous assessment of each patient's situation while maintaining compliance with federal and state regulations. Perform services using approved clinical education and hold a valid state license with at least one year of licensed professional experience. Maintain a valid driver's license, automobile liability insurance, and dependable transportation for all driving conditions. Possess CPR certification for healthcare providers and basic technology skills for mobile device operation. Prioritize Medicare home health or hospice experience.
Required Qualifications
- Must be a graduate of an approved school of clinical education
- Must be licensed in the state where they currently practice
- Must have at least one year experience as a licensed professional
- Must have basic demonstrated technology skills, including operation of a mobile device
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- Must possess CPR certification for the healthcare provider
