Job Summary
Supervise the planning, coordination, and delivery of interdisciplinary group interventions for Hospice services while assigning patients and families to appropriate staff. Lead regular team meetings, review patient care plans, and provide clinical supervision to nursing personnel, Hospice Aides, and Homemakers. Conduct supervisory visits, monitor compliance with policies, and maintain accurate medical records. Deliver direct nursing services, teach families on care provision, and perform symptom management procedures following doctor's orders. Participate in the on-call system, QAPI program, and assist in staff education and orientation. Regularly assess nursing needs and safeguard patient information in compliance with HIPAA regulations.
Required Qualifications
- Graduate from an accredited school of nursing
- Currently licensed as a registered nurse to practice in the state
- Minimum 2 years of experience as a registered nurse
- Must have a current valid driver's license
- Must possess auto liability insurance
- Must have reliable transportation
- Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position
- Must provide evidence of annual TB test
- Must provide evidence of other state-required tests or examinations
- Must be able to work independently
- Must be able to make judgments based on assessments and data available and act accordingly
- Must be flexible
- Must be innovative
- Must possess good interpersonal skills
- Must be able to cope with mental and emotional stress
- Must demonstrate emotional stability
- Must be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials

- Must be able to supervise others appropriately
- Must be able to respond appropriately to crisis outside of a hospital setting
- Must be able to accept and adapt to different social, racial, cultural and religious modes
- Must be able to tolerate exposure to temperature and humidity changes
- Must be able to perform maximal assist in lifting and/or transferring of a 100 pound patient

- Must be able to provide evidence of annual TB test and other state-required tests or examinations
- Must be able to regularly assess and reassess the nursing needs of the Hospice patient
- Must be able to provide Hospice nursing services, treatments and preventive procedures
- Must be able to teach, supervise and counsel the Hospice patient and family members about providing care for the patient
- Must be able to perform specific nursing procedures as needed following doctor's orders
- Must be able to observe confidentiality and safeguard all patient-related information in compliance with HIPAA regulations
- Must be able to communicate to the Director if unable to meet a patient's need or perform a procedure
- Must be able to participate in on-call system and provide on-call coverage when unavailable for assigned duties
- Must be able to maintain skills and knowledge
- Must be able to participate in QAPI program
Desired Qualifications
- Experience as a registered nurse (preferred)
- Active patient contact within past three years (preferred)
