Job Summary
Hospice RN Case Manager in West Palm Beach, FL coordinating comprehensive hospice care with the interdisciplinary care team, assessing patient and family needs, contributing to personalized plans of care, and providing education and guidance to patients and caregivers. Requires FL RN license and BLS; preferred experience in public health, home nursing care, oncology, medical/surgical, geriatric, or hospice nursing. Schedule: Monday–Friday 8:30am–5:00pm with a weekend requirement. Required Qualifications
- Registered Nurse currently licensed in the State of Florida.
- Current Basic Life Support Certification.
- One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
