Job Summary
RN Hospice Case Manager location in Pinellas County, FL. Under the direction of a licensed physician, provides symptom relief and manages discomfort with specialized nursing skills in palliative and end-of-life care. Performs initial and ongoing assessment of physical, functional, psychosocial, and environmental needs; collaborates with the Interdisciplinary Group (IDG) to develop, implement, and update individualized plans of care; initiates referrals and coordinates complementary services as needed; educates patient, family, and caregivers about disease progression, palliative interventions, caregiving, the dying process, and safety practices.
Required Qualifications
- Graduate of approved nursing program
- current unrestricted Florida RN license
- CPR certification required
- minimum one year experience in an acute-care Hospital environment providing direct patient care
- previous hospice RN experience preferred
- CHPN certification encouraged within three years
- BSN or MSN preferred
