Job Summary
Provide comprehensive end-of-life care as an RN Hospice in patients' homes and facilities. Perform assessments, manage pain and symptoms, and educate patients and families while coordinating with an interdisciplinary hospice team. Develop and update individualized care plans, ensure accurate documentation, and advocate for patient comfort and dignity.
