RN Hospice Admission

Educate patients and families regarding hospice services and palliative care to facilitate informed admission decisions. Perform comprehensive head-to-toe assessments, review medication profiles, and determine eligibility based on disease history and current condition. Coordinate interdisciplinary team members, obtain verbal certification from the attending physician and medical director, and develop an individualized plan of care including visit frequencies. Order assistive devices, emergency medication kits, and standard durable medical equipment as needed. Provide transportation to the site of admission and offer emotional support related to anticipatory grief and transition to hospice.
