RN Hospice Admission

Educate patients and families regarding hospice care, admission criteria, and financial benefits to facilitate informed decisions. Conduct head-to-toe assessments, review medical records and medication profiles, and determine eligibility based on disease history and current condition. Obtain verbal certification and initial orders from the attending physician and medical director, then develop an individualized plan of care including visit frequency and assistive device orders. Provide transportation to the site of admission, coordinate resources for patient safety, and introduce the interdisciplinary team. Offer emotional support during the transition to hospice, assist with advance directives, and collaborate on care plans. Requires a Georgia driver's license and minimum two years of acute care and hospice experience.
