Job Summary
Hospice Admission RN needed to educate patients and families about hospice services and palliative care, perform admissions for patients choosing to live at home, ALF, or SNF, and establish the initial plan of care after nursing assessment. Responsibilities include determining hospice eligibility, obtaining verbal certification and admission orders from the attending physician and hospice Medical Director, coordinating with the interdisciplinary team, educating families about Medicare/Medicaid benefits and patient financial responsibilities, conducting head-to-toe assessments focused on terminal illness, reviewing medical records and medication profiles, ordering required assistive devices and emergency kits per protocol, developing individualized visit frequencies, and providing emotional support during transition to hospice care. Requires license as an RN, adherence to the WellStar Practice Model, and compliance with nursing standards. Must be able to transport to admission site and work within the patient/family-centered care framework. Office location for this role is Austell, GA.
Required Qualifications
- Active RN license in Georgia
- BLS - Basic Life Support
- RN - Single State or RN-COMPACT (multistate) license
- Georgia Driver's License or Valid State Driver's License
- Private Vehicle insurance (PVI)
- Minimum 2 years acute care experience
- Hospice experience preferred
- Associates degree or Diploma in Nursing; Bachelor's degree in Nursing preferred
