RN Hospice Admission- Hospice

Hospice Admission RN position at WellStar seeking a licensed nurse to educate patients and families on hospice services and palliative care, perform hospice admissions for patients choosing to live at home, ALF or SNF, conduct nursing assessments to determine eligibility, obtain verbal certification and admission orders, develop initial plan of care, determine visit frequencies, coordinate with the interdisciplinary team, and provide emotional support to patients and families. Requires ongoing nursing competencies, understanding of Medicare/Medicaid benefits, and adherence to professional practice standards. Travel to patient homes for admission and related duties are included; role emphasizes education, assessment, care coordination, and collaboration with the hospice team.
