Job Summary
RN Hospice Admission role providing education to patient and family on hospice services and palliative care, conducting hospice admissions for patients choosing home, ALF, or SNF, performing nursing assessments, determining eligibility, obtaining verbal certification and admission orders, coordinating with the interdisciplinary team, developing the initial plan of care including visit frequencies, and educating families on hospice processes and finances; requires active RN license, BLS, and at least 2 years acute/hospice experience. Operates within WellStar practice standards and supports patient-centered care at admission.
Required Qualifications
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- GADL - Georgia Driver's License or VSDL - Valid State Driver License
- PVI - Private Vehicle insurance
- Minimum 2 years acute care experience and hospice experience
