Job Summary
RN Hospice Admission role Educates patients and families about hospice services and palliative care, performs home/ALF/SNF admissions, determines hospice eligibility, obtains verbal certification and admission orders, and conducts head-to-toe assessments. Develops initial plan of care with the interdisciplinary team, orders necessary devices and emergency kits, determines visit frequencies, educates families about hospice care and finances, and provides emotional support during transition to hospice. Requires active RN license, BLS, state driver’s license, and at least 2 years of acute/hospice experience; role supports home-based hospice admissions in Georgia.
Required Qualifications
- Associates Nursing or Diploma
- RN license (Single State) or RN-COMPACT
- BLS - Basic Life Support
- Georgia Driver's License or Valid State Driver's License
- Private Vehicle insurance
- Minimum 2 years acute care experience and hospice experience
- RN nursing education (Associates/Diploma preferred, Bachelor's preferred)
