Job Summary
Licensed RN in Georgia provides hospice admissions education to patients and families, conducts admission assessments for patients choosing to receive hospice at home, ALF, or SNF, determines eligibility for hospice care, develops the initial plan of care with visit frequencies, obtains verbal certification and admission orders from the attending physician and hospice medical director, coordinates with the interdisciplinary team, educates families on hospice benefits and financial responsibilities, performs head-to-toe assessments and reviews medical records and medications, orders necessary equipment per protocol, and supports the transition to hospice care while complying with applicable policies and standards.
Required Qualifications
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- GADL - Georgia Driver's License or VSDL - Valid State Driver's License
- PVI - Private Vehicle insurance
