Job Summary
Hospice admission RN responsibilities include educating patients and families about hospice services and the palliative approach, evaluating hospice eligibility, conducting nursing admissions for patients choosing to live at home, ALF, or SNF, performing head-to-toe assessments, reviewing medications and comorbidities, developing the initial plan of care with visit frequencies, obtaining verbal certification and admission orders from a physician and hospice director, and coordinating with the interdisciplinary team to support the patient and family through the transition to hospice care. Requires active RN license, BLS, and Georgia driver’s/license; minimum 2 years acute care and hospice experience; willing to coordinate care in home settings and educate families about benefits and responsibilities. The role is based in Roswell, GA with day shift expectations at WellStar Health System in Georgia.
Required Qualifications
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- GADL - Georgia Driver's License or VSDL - Valid State Driver's License
- PVI - Private Vehicle insurance
- Minimum 2 years acute care experience and hospice experience
