RN Hospice Admission - Hospice

Hospice admission RN educates patients and families about hospice services and the palliative approach, performs the admission assessment to determine hospice eligibility, obtains verbal certification and admission orders, develops the initial plan of care with visit frequency, coordinates with the interdisciplinary team, educates on Medicare/Medicaid benefits and patient financial responsibilities, orders necessary equipment, supports the emotional transition for patient and family, and collaborates with the hospice Medical Director as part of the admission process. The role requires home-based or facility-based admission activities, adherence to the WellStar practice model, and commitment to compassionate, evidence-based nursing care.
