RN Hospice Admission- Hospice

Hospice Admission RN needed to educate patients and families about hospice services and palliative care, perform hospice admissions for patients choosing home, ALF, or SNF, conduct nursing assessments to determine eligibility, obtain verbal certification and admission orders, develop initial plan of care, coordinate with interdisciplinary team, and provide guidance and emotional support to patients and families. Requires license as RN, with BLS certification and Georgia driver’s license; 2 years acute care and hospice experience preferred. Location: Vinings, GA.
