Job Summary
As a RN you will provide skilled nursing assessments to participants and work directly with the Interdisciplinary Team, participant, and representative to achieve care-plan goals. You will communicate with family or caregiver and provide appropriate patient education. Must be a graduate of an accredited RN program and licensed in Colorado. This role supports care coordination in home health/homecare settings and requires familiarity with Electronic Health Records, charting, medical equipment, and transportation. The position offers full benefits, a 403(b) with company match, PTO from day 1, weekend/night shift differentials, mileage reimbursement, and ongoing onboarding with preceptor.
Required Qualifications
- Graduate of an accredited Registered Nurse Program
- Licensed as a professional in the State of Colorado
- Healthcare experience in hospital, long-term or home health care setting with 1 year of experience with frail or elderly population preferred
- Reliable transportation and current automobile insurance
- Familiar with computer applications (email, internet, electronic timekeeping, Electronic Health Records)
- Knowledge of medical equipment and instruments
