Job Summary
RN Homecare Field Case Manager coordinates total patient care, evaluates nursing services, and performs admissions, transfers, recertification, resumption of care, and discharge Oasis for the home health patient. The role requires driving to patients’ homes, evaluating care needs, and ensuring adherence to Medicare/Medicaid guidelines. Duties include ongoing patient-care coordination and collaborating with the care team, with responsibilities complemented by documentation support tools and per diem visit opportunities. Requires RN licensure, BLS/CPR, and qualification in home health; standard nursing activities and travel to patient residences are involved.
Required Qualifications
- Graduate from an accredited School of Nursing
- Licensed in the state as a Registered Nurse
- One or more years of experience in community/home health agency or hospital preferred
- Knowledge of Medicare and Medicaid guidelines
- Criminal background check
- Must have a current CPR (BLS) certification
