Job Summary
RN Case Manager - Home Health: Provide comprehensive nursing care and case management for patients in their homes, developing, coordinating, and scheduling individualized care plans with a multidisciplinary team; maintain timely documentation; mentor newly hired nurses; ensure regulatory compliance and high-quality outcomes; utilize EPIC charting, participate in quality improvement, and support through sign-on bonus and relocation benefits. Roles include direct patient care, OASIS assessments as required, and coordination of care across providers and community resources. Requires an Associate's degree in Nursing, NM RN license, BLS, and 1+ year of direct patient care/home health experience. Includes eligibility for tuition/certification reimbursement and various lead roles.
Required Qualifications
- Associates degree in Nursing
- New Mexico RN License
- BLS Required at time of Hire
- Must have current valid driver's license and auto insurance
- Minimum 1 year in direct patient care setting or home healthcare
