Job Summary
Provide compassionate, skilled nursing care in patients' homes as a Home Health RN Case Manager. Develop, coordinate, and schedule individualized care plans through a multidisciplinary team; maintain accurate documentation and serve as a patient advocate. Responsibilities include ensuring quality and regulatory-compliant care, mentoring newly hired nurses, and leading case-management activities across weekdays. Proficient communication with healthcare providers, support services, and community resources is required to deliver cost-effective care and positive patient outcomes. Requirements include an Associate degree in Nursing, NM RN license, BLS, a valid driver's license with auto insurance, and at least 1 year of direct patient care or home healthcare experience.
