Job Summary
Provide compassionate, skilled nursing care in patients’ homes as an RN Case Manager. Develop, coordinate, and schedule individualized care plans through a multidisciplinary team; maintain timely documentation; advocate for patients; mentor newly hired nurses; ensure compliance, continuity of care, and patient satisfaction. Utilize EPIC charting and complete mandated OASIS assessments to meet regulatory standards while supporting quality outcomes and cost-effective care. Required Qualifications
- Associates degree in Nursing
- New Mexico RN License
- BLS required
- Current driver's license and auto insurance
- Minimum 1 year direct patient care or home healthcare experience
